What payment methods do you offer? Can I get a quote/purchase order/payment invoice?

Debit or credit cards (Visa, MasterCard, American Express,...) are accepted. It is also possible to pay with a PayPal account. If your credit card is not accepted by Paypal, we cannot accept your payment at the moment and we are very sorry. We offer PO/invoice-based payment for orders above $2,000.

Do you offer discounts for education or non-profits?

If you’re a non-profit and need more than 100 users, we offer unlimited users for $1,500.

How can I send email notifications from another account?

To send email notifications from another account, you need to transfer the ownership of all Form Publisher settings on your Google Form from you original account to the other account. To do so, please follow the instructions from this documentation article.

Is Form Publisher HIPAA compliant?

We don't have any certification for HIPAA compliance. However, note that Form Publisher fully relies on Google technologies, and Google is HIPAA compliant. We only use Google API and all our actions are based on their servers. Therefore, we are following Google rules and security policies. If you intend to use Form Publisher for any purpose or in any manner involving Protected Health Information, as defined in the HIPAA, it is your responsibility to (a) execute a Business Associate Agreement with Google related to your HIPAA data stored by your in G Suite account (including your Drive), and (b) execute a Business Associate Agreement with Form Publisher related to your HIPAA data stored on Form Publisher, if any. Can I setup a multi-level approval workflow within Form Publisher?

Form Publisher currently allows single-level approval workflow only, but multi-level approval workflow is on our roadmap.
